Can BPJS Health be 1 year in arrears? Can it still be used?
There are rules regarding BPJS Health payments, if you are in arrears with BPJS Health, you will be subject to sanctions. Apart from that, when you try to use a BPJS Health card it is possible that the card will be rejected. This is all due to the BPJS Health arrears.

The participant’s status becomes inactive from the 1st of the following month and health service coverage is temporarily suspended. In the event of arrears and payment has not been made, BPJS Health will not be able to be used for treatment. This means that your BPJS Health is blocked and there are outstanding contributions that must be paid off before it can be active again. Understand that BPJS Health membership can be active again if the participant pays the outstanding monthly contribution of at most 24 months and pays the current month’s contribution. If within 45 days of active membership status again, the participant requires inpatient services, then a service fine will be imposed.
From the explanation above, it can be concluded that BPJS Health which is in arrears for 1 year or more, cannot be used temporarily or is in inactive status. So if you want to reactivate it, you have to pay all BPJS Health arrears.
How to Activate BPJS Health which is in arrears
Reactivating BPJS Health membership status from non-active to active is quite simple.
Head of Public Relations for BPJS Health, Iqbal Anas Ma’ruf, explained that inactive BPJS Health membership can immediately become active again if the participant has paid the outstanding contributions. For example, in January 2022 BPJS Health participants have paid contributions, but in February 2022 BPJS participants have not paid contributions, so in March BPJS Health membership automatically becomes inactive. However, if it has been paid in March, membership will be active again.
After the BPJS Health arrears have been successfully paid, membership will automatically be active again. However, there are fines as regulated. The amount of the fine is as follows:
The amount of the fine imposed is five percent of the estimated treatment costs according to the results of the initial diagnosis and procedures for each month in arrears. The provisions for this fine are that the maximum number of months in arrears is 12 months and the maximum amount is IDR 30 million. This is stated in Article 42 Paragraph (6) of Presidential Decree Number 64 of 2020.
In fact, participants who are in arrears or are late in paying BPJS Health contributions are not charged any fines at all. However, according to Presidential Regulation Number 64 of 2020, membership in arrears of contributions will be suspended or temporarily frozen from the 1st of the following month. When can BPJS be used after arrears?
Before paying arrears, you can check arrears in the following way:
Check bills via the official BPJS Health website at https://bpjs-kesehatan.go.id/ Visit government bank ATMs (BNI, Mandiri, BRI). Select the BPJS Health bill and enter the BPJS Health Virtual Account (VA) number (obtained when registering the account). Download the BPJS Health application on PlayStore. Contact the BPJS contact service at 1500 400 or the BPJS Health Mobile Customer Service (MCS).
After that, you can make payment of arrears. To pay outstanding contributions between 1 to 6 months, this can be done at an ATM, post office, Indomaret or Alfamart. However, if you are in arrears for 1 year or more, payment and fines can only be made through the nearest BPJS Health office.
Once the arrears are paid off, your membership status will be reactivated and you can use BPJS services for treatment, except for inpatient treatment.
If you want to use inpatient care, it is recommended to wait 45 days after the status is reactivated, because if you use inpatient facilities during the 45 day window, you will be charged 5% of the inpatient costs.

The installment program only applies to Non-Wage Recipient Workers (PBPU) and Non-Workers (BP). Participants have a minimum of 4 months in arrears and a maximum of 24 months. The maximum installment payment stage in 1 program cycle is 12 months. Participants must register via the Mobile JKN application or BPJS Health Care Center 165. Later, membership status will become active again after the current month’s arrears and contributions are paid in full.
This arrears payment also takes into account a family’s arrears. This means that REHAB program participants no longer need to register for the REHAB program for each family member.
